Coverage built around short-term in-home relief care and the unsupervised one-to-one contact it requires.

What underwriters look at

Respite care providers step in for short periods to give family caregivers a break, typically working alone inside a client's home with little to no direct oversight during the visit. That structure, one caregiver, one client, no co-worker present, creates a distinct exposure profile: if a client is injured, medicated incorrectly, or alleges mistreatment during a respite visit, there are often no witnesses beyond the caregiver and the client, which puts significant weight on documentation and training.

Because visits happen in the client's own home rather than a licensed facility, respite caregivers encounter unpredictable physical environments, uneven flooring, unfamiliar medical equipment, pets, that a facility-based provider would not face in the same way. Driving clients to appointments or errands during a respite shift adds an auto exposure that many providers underestimate, since it involves both the caregiver's vehicle and, in some cases, the client's own car.

Respite care is frequently funded through Medicaid waiver programs, area agencies on aging, or veteran and disability benefit programs, each of which typically requires the provider to carry specific liability limits and list the funding source as an additional insured on the policy. Our Live Certificate program allows respite agencies to generate compliant certificates on demand whenever a funding source or referral partner requests updated proof of coverage.

Unsupervised One-to-One Contact

Respite visits typically involve a single caregiver alone with a client, meaning allegations of mistreatment or negligence often come down to documentation with no witnesses present.

In-Home Environmental Hazards

Uneven flooring, unfamiliar medical equipment, and other conditions specific to a client's own home create injury risks a facility-based provider would not face.

Client Transport Exposure

Driving clients to appointments or errands during a visit introduces auto liability exposure involving both the caregiver's and sometimes the client's vehicle.

Medication and Care Task Errors

Caregivers assisting with medication reminders or basic care tasks without on-site nursing oversight can make errors that lead to professional liability claims.

What it typically costs

Respite care insurance costs generally reflect the number of active caregivers, how much client transport occurs during visits, and the acuity of clients served. Agencies serving clients with complex medical needs typically see higher professional liability costs than those offering companionship-level care.

Business sizeWhat drives the cost at this size

Small provider (under 10 caregivers)

Reflects a limited client roster and modest visit volume.

Mid-size provider (10–30 caregivers)

Higher visit volume and transport activity increase liability exposure.

Large provider (30+ caregivers)

Scale and higher-acuity clients raise premiums further.

Pricing is set by each carrier and varies by state, limits, payroll, and loss history — this is not a quote.

What moves your premium
